I'd guess a vacuum or unmetered air leak, perhaps engine mounts, then head gasket.
Check for an unmetered air leak:
using a B bottle, LPG, or even a carb cleaner spray any areas where a vacuum leak or unmetered air leak could occur. You may want to disconnect the O2 sensor so the engine is more 'sensitive' to the gas/spray. When the engine smooths out you found the leak. Also check the condition of all rubber hoses, tubes, and lines on the vacuum side of the engine. If they are soggy or brittle replace them.
Check for excessive oil or crackes at each engine mount.
Look for cream in the oil or the oil cap, oil residue or even cream in the coolant, or bubbles in the coolant when the engine is running. If the radiator hoses get stiff right away when you start the engine (5 to 15 minutes is normal) or you have white exhaust smoke when the engine is completely warmed up you have a problem head gasket.

A buddy had a similiar problem, turned out to be a crack in his air-box.
