Yep, thats not a lug nut failure, thats either a C-clip or bearing failure and the wheel is still bolted to the axle and the drum

I was thinking more along the lines of a Samurai's backing plate... 4 bolts to hold the whole axle assembly into the housing.

I have had that happen to the Dana 70 in my old 79 F-350 the axle nuts backed off and the hub and wheels walked off the spindle. Its a common problem with full floaters when the lock washer lets go.

I vote on bearing failure as it looks pretty stock and I doubt the diff has ever been touched, it is a 3rd member rear though.. I doubt the backing plate nuts just backed off, if they did it was because they broke..
